This policy covers this website only. How we use and protect your health information as a patient is described in our Notice of Privacy Practices. You can also ask for a paper copy at the front desk or by calling 330-721-1350.
Information you give us
If you send an appointment request, we collect your name, phone number, whether you have been to our office before, the days and times that suit you, and your email address and referring dentist if you give them. We use it only to contact you and schedule your visit. Please do not include health details in the form; we will discuss your tooth by phone.
Information collected automatically
We use Google Analytics to understand how people find and use this website. It sets cookies and receives information such as pages viewed, approximate location, and device type. Advertising features are switched off, and nothing you type into a form is sent to it. Our host, Cloudflare, processes technical information such as IP addresses to deliver pages securely and counts visits without cookies.
Your choices
Use Your privacy choices at the bottom of any page to turn Google Analytics off. Your choice is saved in that browser, so make it again on another device or after you clear your browsing data. If your browser sends a Global Privacy Control signal, we treat it as an opt-out automatically. We do not respond to the older Do Not Track signal. We have turned off the Google Analytics features that connect your visits here with other websites and devices.
What we do not do
We do not sell personal information, share it for advertising, or use advertising pixels on this site.
Patient and referral portals
The patient portal and the referral portal are on a separate secure site run by our practice software provider, not on this website. What you enter there becomes part of your patient record and is protected as described in our Notice of Privacy Practices.
